Section 01
Information we collect
We collect the following personal information through the Site and our phone line:
- Contact details such as your name, email address, and phone number, when you submit the contact or provider intake form.
- Professional details such as practice name, provider type, state of practice, and National Provider Identifier (NPI), when you submit the provider intake form.
- Message content, including the subject, your role, and anything you choose to write in a form.
- Lot numbers you search in the lot lookup.
- Technical data such as your IP address, browser type, device and operating system, pages visited, referring page, and the date and time of your visit, kept in server logs. Your IP address may indicate an approximate city or region. The Site does not request your device location.
- Phone call recordings, when you call the pharmacy, as described in Section 04.
How we collect your information
- From you directly, when you submit a form, search a lot number, email us, or call us.
- Automatically from your browser when you visit the Site.
Section 02
How we use information
We use personal information to:
- Respond to your messages and inquiries.
- Review provider inquiries, including verifying a prescriber's license and NPI.
- Operate, secure, and improve the Site, including preventing spam and abuse of our forms.
- Review the quality of our service and train our staff, using call recordings.
- Comply with our legal obligations, including Texas State Board of Pharmacy rules and HIPAA.
- De-identify information as permitted by law.
We do not sell your personal information, and we do not use it for targeted advertising.
Section 03
Website forms and health information
The Site's forms are not a channel for protected health information, and form submissions do not become part of your pharmacy record. Please do not include medical details, prescription information, or other health information in any website form. For anything about your health or your prescriptions, call the pharmacy at +1 (844) 202-4446.
Form submissions are delivered to our staff by email. If a submission includes health information, we use it only to respond to you, and we will ask you to call the pharmacy.
Section 04
Phone calls
Calls to the pharmacy may be recorded for quality and training. We use recordings to review the accuracy and quality of our service, to train our staff, and to resolve questions about what was said on a call.
Only authorized staff can access recordings. We keep a recording only as long as needed for these purposes, unless the law or a pending dispute requires us to keep it longer. When a call involves your health or your prescriptions, the information discussed is protected health information and is also covered by our HIPAA Notice of Privacy Practices.
Section 05
Cookies and advertising
The Site does not set any cookies of its own. We do not use advertising or analytics cookies, and we do not use advertising networks, tracking pixels, or web beacons.
Our Contact page embeds a Google Maps map. When the map loads, Google may set its own cookies and collect information such as your IP address under the Google Privacy Policy.
You can block or delete cookies in your browser settings. The Site works the same with cookies blocked.
Section 06
Who receives information
We share personal information only as needed with:
- Website hosting (Microsoft Azure).
- Email delivery of form submissions (Resend).
- Spam and abuse protection, which briefly stores your IP address (Upstash).
- Maps (Google Maps), when the map on our Contact page loads.
- Professional advisers, regulators, and law enforcement, where required by law.
- A buyer or successor in a business transaction.
We may also share personal information with your consent or at your direction.
Section 07
Data retention
We keep personal information only as long as needed for the purposes in this Privacy Policy:
- Form submissions: as long as needed to respond to you and for our business records.
- Server logs: for a limited period, for security and troubleshooting.
- Spam protection records: about 15 minutes.
- Call recordings: as described in Section 04.
- Pharmacy records: as long as Texas State Board of Pharmacy rules and HIPAA require.
Section 08
Security
We use encryption in transit (HTTPS), access controls, and reputable hosting providers. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.
Section 09
Your choices and rights
We do not sell your personal information, and we do not use it for targeted advertising or profiling. You may ask to access, correct, or delete personal information we hold from the Site by emailing privacy@greenwichrx.org. We will verify your identity before acting on a request, usually by confirming details you gave us, and we will respond within 45 days. You may also request a copy of your personal information in a portable format. We may not be able to delete information the law requires us to keep.
If we decline your request, we will explain why, and you may appeal by replying to our decision at the same address. Requests about your pharmacy record follow our HIPAA Notice of Privacy Practices.
Section 10
Children
The Site is not directed to children under 13, and we do not knowingly collect personal information from them through the Site. A parent or guardian should contact the pharmacy on a child's behalf. If you believe a child has sent us information, contact privacy@greenwichrx.org and we will delete it.
Section 11
Links and social media
The Site links to our LinkedIn page and to other websites. Their own privacy policies apply once you go there, and we encourage you to read them.
Section 12
Changes to this Privacy Policy
We may update this Privacy Policy. When we do, we will post the new version on this page and change the "Last updated" date. If we make a material change to how we use information we already hold, we will ask for your consent where the law requires it.
